Adding Check box and Prefilled text

How do we add a check box that we can pre check before sending for signature? Also. Where do we add text to a text box in the form? We are using Blend company for our loans.

Thanks for your post! Generally speaking, if you are designing the document layout with Designer Page, check box field and label field could serve for your use case:
However if you are leveraging an integration implemented by Blend, it could be different from the normal UI flow and hence it could also help to reach out to Blend's support team for the best practices to use their application.
